2027 Prostate Biopsy CPT Updates

What Urology Providers Need to Document · Effective January 1, 2027

Prostate biopsy coding changes again in 2027. Following the 2026 restructuring, the updated codes more clearly distinguish regional sampling, fusion-targeted sampling, and procedures that include both.

For providers, the priority is clear documentation of the approach, guidance technique, sampling performed, and distinct lesions targeted.

WHAT IS CHANGING?

Prostate biopsy code changes effective January 1, 2027
Code(s)2027 ChangePractical Impact
55706DeletedSeparate transperineal stereotactic template-guided saturation biopsy code retired.
55712DeletedTransperineal fusion-targeted-only biopsy moves to revised 55711.
55713DeletedSeparate in-bore regional-plus-targeted biopsy code retired.
55707–55710RevisedClarifies regional sampling and regional plus fusion-targeted sampling by approach.
55711RevisedFirst fusion-targeted lesion without regional sampling; either approach.
+55716NewEach additional fusion-targeted lesion without regional sampling, with 55711.
55714 / +55715RevisedIn-bore CT/MRI-targeted biopsy: first lesion and each additional lesion.

Descriptions are abbreviated educational summaries, not official CPT descriptors.

MATCH THE DOCUMENTATION TO THE PROCEDURE

Select the pathway supported by the procedure note
Procedure Documented2027 Code Pathway
Transrectal regional biopsy55707
Transrectal regional and fusion-targeted sampling55708
Transperineal regional biopsy55709
Transperineal regional and fusion-targeted sampling55710
Fusion-targeted-only biopsy, either approach55711 + 55716 for each additional targeted lesion
In-bore CT/MRI-targeted biopsy55714 + 55715 for each additional targeted lesion

Key distinction: The new +55716 pathway applies to additional fusion-targeted lesions with 55711, when regional sampling is not performed. The revised +55715 pathway applies to additional in-bore CT/MRI-targeted lesions. Do not automatically carry 2026 add-on reporting into 2027.

BASIC DOCUMENTATION REQUIREMENTS

Your procedure note should clearly identify:

  • Approach: Transrectal or transperineal.
  • Guidance: Ultrasound, MRI-ultrasound fusion, or in-bore CT/MRI.
  • Sampling: Regional, fusion-targeted only, or both.
  • Targets: Number and location of distinct targeted lesions.
  • Specimens: Cores obtained from each target and regional sampling, when performed.

Document lesion count separately from core count. Multiple cores taken from one lesion should not be described as multiple distinct targets.

PREPARE YOUR PRACTICE

Review biopsy templates, order sets, charge tickets, and coding workflows before January 1. Remove deleted codes and ensure the documentation supports the revised procedure categories. Imaging guidance is included in the listed image-guided biopsy descriptions; review CPT instructions before separately reporting associated imaging services.

Final code selection should follow the 2027 CPT descriptors and parenthetical instructions, applicable NCCI edits, and payer requirements. CPT changes do not independently establish coverage or payment.
